Health Ministry Launches Digital App to Strengthen Primary Healthcare Services Nationwide

New Delhi : In a major initiative to enhance primary healthcare delivery and expand digital health services, the Ministry of Health and Family Welfare has launched a dedicated mobile application aimed at supporting Community Health Officers (CHOs) across the country by streamlining their clinical workflow and decision-making processes.

The application, developed by the Indian Council of Medical Research, is designed as a comprehensive clinical decision-support system and job-aid tool. It will be used by CHOs working at Sub-Centre Ayushman Arogya Mandir facilities, aligning with the expanded 12 service packages under Comprehensive Primary Health Care (CPHC).

The platform offers structured clinical workflows that act as quick-reference checklists for managing patients visiting outpatient departments at these centres. It provides step-by-step guidance for patient assessment, including history-taking, physical examinations, and recommended diagnostic procedures based on symptoms and presenting complaints.

One of the key features of the application is its ability to assist CHOs in identifying cases that require immediate referral to higher-level healthcare facilities. It also supports pre-referral management to stabilise patients in critical conditions. For non-emergency cases, the tool enables informed clinical decisions by suggesting appropriate treatments, referral pathways, or teleconsultation with specialists.

The application is organised into three main sections—Workflows, Assessment Tools, and Treat & Counsel—making it user-friendly and efficient in real-time clinical settings. A colour-coded system has been integrated to simplify decision-making: Red for life-threatening conditions requiring urgent referral, Orange for cases needing specialist consultation, Yellow for manageable conditions at the facility level, and Green for mild or routine cases.

In addition to clinical support, the app integrates several digital health features, including patient registration, electronic health records (EHR), teleconsultation services, diagnostic reporting, and follow-up tracking. It is also designed to seamlessly integrate with existing national digital health platforms, ensuring continuity of care and improved health outcomes.

Addressing the launch event, V. K. Paul, Member (Health) at NITI Aayog, emphasized the foundational role of primary healthcare in strengthening the overall health system. He stated that India’s progress towards Universal Health Coverage is deeply rooted in robust primary healthcare infrastructure.

Dr. Paul noted that the launch coincides with the completion of eight years of the Ayushman Arogya Mandir initiative, marking an important milestone. He welcomed the introduction of this practical digital tool, stating that it would significantly empower CHOs across the country. He also highlighted the importance of continuous feedback mechanisms to refine the application and stressed the need to integrate Artificial Intelligence (AI) capabilities in the future to further enhance clinical decision-making and service delivery.

Speaking on the occasion, Rajiv Bahl, Secretary, Department of Health Research and Director General of ICMR, described the launch as a crucial step in strengthening digital clinical support at the grassroots level. He stated that the application would evolve continuously based on field usage and feedback from healthcare providers.

Dr. Bahl appreciated the collaborative efforts involved in developing and evaluating the application, noting that such evidence-based and user-centric approaches are vital for building effective digital health solutions. He reaffirmed ICMR’s commitment to further improving the platform to better serve frontline healthcare workers.

This innovative, evidence-based digital tool is expected to significantly enhance the quality, consistency, and timeliness of healthcare services at the primary level, while equipping Community Health Officers with standardized and accessible clinical protocols.
